What materials do I need to apply?

Materials will vary based on whether you’re applying for a first or second PPP loan. Please see MoFi’s application forms for a complete list of materials needed to apply:

  • For first-time PPP clients who have not received a PPP loan: https://formstack.io/9B9B5
    • NOTE: At the formstack link above, you’ll be asked to upload the SBA’s PPP application. (Form 2483-C for Schedule C filers or Form 2483 if you don’t file on a Schedule C)
  • For second-time PPP clients who received their first PPP loan from MoFi in 2020 or 2021: https://formstack.io/53A50
    • NOTE: At the formstack link above, you’ll be asked to upload the SBA’s PPP application. (Form 2483-SD-C for Schedule C filers or Form 2483-SD if you don’t file on a Schedule C)
  • For second-time PPP clients who received their first PPP loan from a different lender: https://formstack.io/0C7A3
    • NOTE: At the formstack link above, you’ll be asked to upload the SBA’s PPP application. (Form 2483-SD-C for Schedule C filers or Form 2483-SD if you don’t file on a Schedule C)
